Position: Nurse – Post-cardiac surgery follow-up

Team Heart, Inc. is a non-governmental international organization seeking to address the burden of cardiovascular disease in Rwanda through early prevention and screening, expanding access to care through a collaborative data driven approach, improvement of infrastructure and facilities, skill transfer, and access to essential cardiac medications and supplies for all. Team Heart, Inc. has over 17-years of experience providing cardiac care including surgery to the most vulnerable Rwandans while also building the capacity of Rwanda’s health system and providers to expand access to quality cardiac care around Rwanda.
Team Heart is therefore seeking an enthusiastic nurse to support in post-cardiac surgery follow-up and ensure continuity of care for patients recovering from heart surgery.
Position responsibilities
Patient registration and data management
- Register post-operative patients into the Redcap registry system.
- Ensure timely and accurate data entry, documentation, and reporting.
Clinical follow-up & monitoring
- Work with NCD nurses based in hospitals working with Team Heart, Inc. to monitor patient health status post-discharge when needed.
- Conduct routine clinical follow-up visits at district hospitals in collaboration with local healthcare teams (NCD nurses).
- Identify and address potential complications promptly, ensuring timely referrals or interventions.
Collaboration with district & referral hospital NCD Teams
- Work closely with NCD nurses at district and referral hospitals to ensure smooth handover and local follow-up of patients.
- Link patients to NCD clinics for long-term care and support.
- Facilitate training or case discussions with NCD nurses to enhance continuity of cardiac care.
Patient and family engagement
- Maintain regular communication with nurses, patients and their families to ensure adherence to treatment plans and follow-up appointments.
- Provide education on post-surgical care and treatment adherence
- Advocate for patients facing social, economic, or access-related barriers.
Reporting and documentation
- Develop regular follow-up reports.
- Document patient progress, clinical findings, and identify barriers to care.
- Maintain confidentiality and ensure accurate tracking of patient outcomes.
Team coordination
- Collaborate closely with the Team Heart clinical and administrative teams, as well as other stakeholders, including King Faisal Hospital and Rwanda Biomedical Centre.
- Participate in planning meetings, case reviews, and multidisciplinary discussions.
- Serve as ad hoc member and advisor to Rwanda Cardiac Post-Operative Network
- Perform additional tasks as assigned to support patient care and program goals.
Qualifications and skills
- Minimum of a bachelor degree in nursing with a valid license issued by National Council of Nurses and Midwives.
- Minimum of 3 years of relevant professional experience as a NCD nurse or CVTS-ICU theatre at King Faisal or other facility providing cardiac surgery.
- Skills and expertise in developing concept notes proposals, reports, and presentations.
- Office computer and data management skills with proficiency in MS Office (World, Excel, and PowerPoint).
- Prior experience with Redcap or other patient tracking data program is an added advantage.
- Willingness to be part of large team, flexible, collaborative consider different perspectives and work from multiple locations.
- Ability to communicate in both English and Kinyarwanda
